The Frank C. and Lynn Scaduto MIND Institute at Miami Jewish Health is proud to be a participant in Medicare’s Guiding an Improved Dementia Experience (GUIDE) program. This innovative model allows us to provide comprehensive dementia care and support services designed to improve quality of life for people living with dementia and their family caregivers.

Through our GUIDE program, you’ll receive personalized care coordination, education, 24/7 support, and access to respite services – all at no cost to eligible participants. Our compassionate team is committed to helping you and your loved one navigate dementia with dignity, support, and expert care.

What is the GUIDE Program?

The GUIDE program is a revolutionary approach to dementia care. It provides a dedicated team, led by a dementia care specialist, to go beyond traditional medical treatment to address the complete needs of both the person with dementia and their caregiver.

Enrollment Support

When it comes to dementia care, we know you have a lot on your plate. We manage all paperwork and eligibility verification, seamlessly enrolling patients and caregivers in the GUIDE program so they can access benefits as soon as possible.

Dedicated Care Navigator

You will be assigned a Care Navigator who will be your main point of contact, helping you access services and coordinate care.

Comprehensive Care Plan

We develop an individualized plan that addresses everything from medical needs to daily living and future planning.

24/7 Support

You have around-the-clock telephone access to our support line for any questions or urgent needs that may arise.

Caregiver Training and Support

We equip caregivers with the education, skills, and resources needed to provide the best possible care as well as manage their own well-being.

Respite Services

The GUIDE program provides up to $2,500 per year for respite care, giving caregivers a much-needed opportunity to rest and recharge.

Am I Eligible for the GUIDE Program?

You may be eligible if you meet all of the following criteria:

You have been diagnosed with dementia at any stage by a clinician

You are enrolled in Medicare Parts A and B, with Medicare as your primary payer

You are not enrolled in Medicare Advantage, Special Needs Plans, or PACE

You are not currently receiving the Medicare hospice benefit

You are not living in a long-term skilled nursing facility

Eligibility Review & Consultation

Our team will evaluate Medicare status, clinical fit, and confirm your diagnosis and eligibility. We handle all the paperwork for you.

Intake & Baseline Assessment

We will conduct a comprehensive evaluation and develop an individualized care plan for eligible participants.

Ongoing Engagement

You and your care team will meet regularly, adapt the plan, and access services as needed.

Care Navigation & Support

Caregivers have 24/7 access to navigators, respite, referrals, and education services.

Frequently Asked Questions (FAQ)

Can you join GUIDE if you are enrolled in Medicare Advantage?

No. GUIDE is currently available only to those with Original Medicare (Parts A & B).

What if the patient is moved into a nursing facility later?

Long-term stay in a skilled nursing facility typically disqualifies the patient from GUIDE.

Do caregivers have to pay for GUIDE services?

No. Caregiver support, education, and access to the care navigator are included in GUIDE services for eligible participants.

Will this limit my access to other doctors or services?

No. You maintain the freedom to see any Medicare-accepting provider. GUIDE supplements your existing care – it does not replace it.

How much respite is available?

Respite is available for up to $2,500 annually to be put toward in-home, adult day care, or short-stay services.
